Citizen Complaint Process

When you have a complaint or concern about the performance of any employee of the Fairfax Police Department you have the right to make a complaint and express your concerns.  Forms/policies/procedures are also available at the Fairfax Police Department.

Traffic Collision Report

If your vehicle was involved in an accident, and you need a report for insurance use, download and use this form.

House/Vacation Watch

To request a House/Vacation watch while you are away from you home for any reason, you will need to fill out a form and submit it to the Fairfax Police Department (in person, fax or by mail or email). As time permits, members of the police department will check the perimeter of your home while you are away.

Temporary Street Closure (Block Party)

Planning a block party for your neighborhood? This is the form you need to prepare and deliver to the Fairfax Police Department. Please make sure to turn in the form at least ten (10) days prior to the event.

Merchant Parking Pass

Merchant Parking Passes allow eligible vehicles to park in designated areas during business hours. Permits are issued annually and must be properly displayed.

Permit Fees

  • $120 per year
  • Fees are prorated for late applications
  • No refunds for surrendered permits
  • No discounts for second vehicles
  • Replacement permits for lost, stolen, or damaged passes are available for $15
  • A 3% processing fee applies to all credit card transactions

Permit Display & Rules

  • Permits must be hung from the rear-view mirror
  • Permits not properly displayed may be subject to citation
  • Permits may not be copied

Where & When You May Park

Merchant Parking Permits are valid:

  • 9:00 a.m. – 6:00 p.m.
  • In the following locations (space permitting):
    • Parkade Parking Lot
    • Mono Lot
    • Lower Pavilion Lot (Elsie to Bank, street level)
    • Other 4-hour designated parking areas

All vehicles must continue to follow all other posted parking rules, laws, and regulations.

How to Apply

To purchase your 2026 Merchant Parking Pass, visit the Fairfax Police Department at 142 Bolinas Road, Fairfax.

A completed application and payment are required at the time of purchase.
